Friar Laurence decided to fake Juliet's death to stop the marriage of her and Paris. Friar Laurence sent a letter Romeo telling him the plan to fake Juliet's death, though the letter never got to Romeo. Romeo found out from Balthasar that Juliet has died. Romeo then went to her tomb to see her and kill himself because he doesn't want to live without her. If Friar Laurence were their to tell Romeo that her death was faked they could have both lived.
This was a bad decision because it causes sorrow throughout the Montague family. Making bad decisions on impulse was how Romeo and Juliet killed themselves. Ignorance causes these rash decisions for other reasons.
